虫垂切除後の縫合糸を核とした膀胱周囲膿瘍

Paravesical abscess caused by silk suture at appendectomy

患者は36歳，女性。急性リンパ性白血病の治療中に尿路感染症を発症した。同院泌尿器科にて精査を施行したところ，尿膜管膿瘍が疑われ手術を施行した。絹糸と思われる人工物周囲に沿って炎症細胞の波及を認めたため，22年前に施行した虫垂切除術の縫合糸膿瘍と診断された。本症例は化学療法による免疫力の低下により，膀胱周囲膿瘍を発症した可能性が示唆される。今後，同様の症例に対し十分な注意が必要である。

A 36 year-old female,under treatment of acute lymphocytic leukemia,complained of general fatigue and lower abdominal discomfort. Investigation of cystoscopy,CT and MRI led the suspected diagnosis of pyourachus,therefore we performed an operation for pyourachus,but pathological diagnosis was suture thread abscess of appendectomy 22 years before. Her post-operative condition was good.
