甲状腺癌再発と診断された頸部デスモイド腫瘍の1例

A Case of Desmoid Tumor Diagnosed as Recurrence of Thyroid Cancer

　はじめに

　デスモイド腫瘍は侵襲性線維腫症（aggressivefibromatosis）とも呼ばれ，全身の筋肉や腱筋膜より発生する比較的稀な線維性腫瘍である^1）。本腫瘍は組織学的には良性であるが周囲組織に浸潤性に増大し，また術後に再発しやすいことから悪性腫瘍に準じた治療が必要とされている^2）。しかし，本腫瘍の術前診断は困難なことが多い。

　今回，過去に摘出された甲状腺左葉の位置に発生し，術前に甲状腺癌再発と診断された頸部デスモイド腫瘍の1例を経験したので報告する。

A 64-year-old woman, who underwent resection of an anterior neck tumor 20 years ago, presented with a neck mass. CT and MRI demonstrated the mass in bothsides of the thyroid. One in the right lobe was a round nodule, the other completely occupied the left lobe. We also found the left vocal cord fixed. She was diagnosed as having recurrence of thyroid cancer, and underwent surgical treatment. The tumor in the left lobe was invad-ing into surrounding tissue, and radical excision includ-ing cervical esophagus was performed. Histologically, the diagnosis of the resected specimen was desmoid tumor.
