食道異物を疑わせた縦隔気腫の1症例

Mediastinal Emphysema Diagnosed as Esophageal Foreign Bodies

　I．緒言

　食道あるいは咽喉頭気管異物の症例においては乳幼児など自意識に欠け，あるいは意志の発表のできないものは別として，何れも必ず，局所の自覚症状の他に誤飲を主訴としている。そのため誤飲の事実を訴えて来院する患者に相対する時，とくに患者が成人である場合はなおさら，当方もまず異物患者として対応する場合が多い。最近誤飲を主訴として来院した患者に，食道直達鏡を予定し，その前処置から心膜炎が疑われ，さらに経時撮影した胸部X線写真の比較から，主因が縦隔気腫であることが判明した症例を経験したので，ここに報告する。

　A 23-year-old man was admitted to our hospital because of the suspicion of an esophageal foreign body. According to the esophagography and ECG, he had first been diagnosed as having pericarditis. Finally we made the diagnosis of mediastinal emphysema by the compariable studies of thoracic radiological findings. Patient with mediastinal emphysema who first visit otorhinolaryngologists is very rare.
