耳掻きによるアブミ骨底板骨折の1例

Fracture of the Stapes Footplate by Earspoon

　はじめに

　外耳道からの中耳外傷は日常臨床においてしばしば経験する疾患であり，中でも耳掻きによるものは，鼓膜穿孔による耳出血，ときには耳小骨離断による難聴を訴えて耳鼻咽喉科を受診する。しかし，そのほとんどは中耳までにとどまるものであり，外リンパ瘻など内耳に及ぶことは比較的少ない。

　今回われわれは耳掻き外傷がアブミ骨底板骨折を引き起こした症例を経験し，アブミ骨手術を施行した。この症例はめまいを伴わず，さらにdry vestibleであった特異な臨床症状を有していたので報告し，文献的考察を加えた。

A case of fracture of the stapes footplate without ossicular disruption of the incudo-stapedial joint induced by an earspoon is reported. A 29-year-old female presented with a hearing loss without ver-tigo after picking in her right ear. Pure tone audio-grams showed a hearing loss of 42.5 dB preope-ratively.

On exploratory tympanotomy, fracture of the stapes footplate without dislocation of the incudo-stapedial joint was observed. The fractured foot-plate was removed and the ossicular chain was reconstructed with Teflon piston.

After the operation, improvement of hearing (conductive loss and air-bone gap) was obtained.
