単脚アブミ骨と線毛細胞分布異常を示した先天性アブミ骨固着症の1例

A Case of Congenital Stapes Fixation with Monopodal Stapes and Abnormal Distribution of Ciliated Cells

　はじめに

　ヒト中耳粘膜における線毛細胞の分布については，これまでにいくつかの報告があり，いずれも線毛細胞は耳管口および耳管口により近い部位に密に分布し，アブミ骨の周辺では線毛細胞の分布を認めないとする報告が大多数である^1）。今回，われわれは両側伝音性難聴と耳鳴を主訴に来科した35歳の男性に試験的鼓室開放術を施行し，アブミ骨の固着を認めたためアブミ骨手術を行った症例を経験した。術後聴力改善と耳鳴の軽快をみたが，摘出されたアブミ骨は閉鎖孔のない単脚アブミ骨を呈し，走査型電子顕微鏡（SEM）による検索で脚表面粘膜に線毛細胞の分布を認めたので若干の文献的考察を加えて報告する。

Exploratory tympanotomy for a 35-year-old male with conductive deafness revealed a mono-podal fixed stapes. Inadvertent stapedectomy wasperformed with sufficient hearing improvement. The SEM study confirmed a columella type mal-formed stapes with abnormal distribution of ciliated cells in its crus.

Instead of the indication of stapedotomy, stapedectomy may be inevitable in the surgery for this type of congenital anomaly. Furthermore, dis-tribution of ciliated cells in the human middle ear cavity was also reviewed.
