Exploratory tympanotomy for a 35-year-old male with conductive deafness revealed a mono-podal fixed stapes. Inadvertent stapedectomy wasperformed with sufficient hearing improvement. The SEM study confirmed a columella type mal-formed stapes with abnormal distribution of ciliated cells in its crus.

Instead of the indication of stapedotomy, stapedectomy may be inevitable in the surgery for this type of congenital anomaly. Furthermore, dis-tribution of ciliated cells in the human middle ear cavity was also reviewed.
