　はじめに

　かつて国民病といわれた結核は，抗結核剤の進歩と公衆衛生の向上によって感染症の主役としての地位を失った。しかし，HIVを初めとする免疫不全状態者への感染や抗結核剤耐性菌の登場によって再び脚光を浴びるようになってきている^1,2）。耳鼻咽喉科領域でも結核性リンパ節炎や喉頭結核を経験することは決して稀ではない^3,4）。しかし，鼻腔結核はそれらに比べ少なく，報告も散見されるに過ぎない。

　今回われわれは，肺に活動性病変を認めない原発性鼻腔結核症例を経験したので報告する。

A 39-year-old female presented with primary nasal tuberculosis. She complained of left nasal obstruction. Local examination revealed white nec-rotic granulation tissue in the left nasal cavity. Thenasal biopsy specimen showed infiltrates of epith-eliod cells and Langhans giant cells in caseous granuloma, suggesting tuberculosis granuloma. The tubercle bacillus DNA was detected by polymerase chain reaction analysis. The chest x-ray showed no abnormal findings. Therefore, the lesion was diagnosed as primary nasal tuberculosis. Combined chemotherapy with REP, INH, EP, PAZ, was ad-ministed. Six months later, the disease disappeared without relapse.
