血液透析患者に発症した原発性喉頭結核の1症例

Primary Tuberculosis of the Larynx in Hemodialysis Patient

　はじめに

　近年，再興感染症として結核が世界的に注目されている。本邦においても新規登録患者数結核罹患率ともに上昇傾向にある^1〜3）。喉頭結核は肺結核に続発するものがほとんどであり，結核患者の増加に伴い再度報告が散見されるようになってきた^3〜10）。近年の傾向としては，症状が軽微で肉芽腫型のものが増加し，喉頭癌との鑑別が重要となってきている^3〜7,10）。

　今回，血液透析中に喉頭結核を発症し，喉頭癌との鑑別の点で診断に苦慮した1症例を経験したので報告する。

A 63-year-old man on hemodialysis presented withtuberculosis of the epiglottis.

The laryngeal lesions were primarily considered to be highly suspicious for neoplastic process rather than infectious one. The final diagnosis of tuberculosis was made by PCR test for tubercle bacillus from biopsied materials. We emphasised the importance of PCR test of biopsied material as well as histopathological examina-tion for the differential diagnosis of laryngeal tumor.
