口蓋扁桃に転移をきたした肺腺扁平上皮癌症例

Tonsillar Metastasis from Adenosquamous Carcinoma of the Lung

　はじめに

　口蓋扁桃に発生する悪性腫瘍は原発性がほとんどであり，転移性腫瘍は極めて少ないといわれている^1〜2）。

　今回われわれは，肺腺扁平上皮癌を原発巣とする転移性扁桃癌の1例を経験したので報告する。

A case of tonsillar metastasis of adenosquamous carcinoma in 2 years after treatment of primary lung cancer was reported. A 69-year-old man was complaining of blood-mixed-saliva. Oral examina-tion revealed that the upper pole of the right palatine tonsil was coated with reddish fur-likecovering. Hemangioma or other benign disease was suspected, but histological examination revealed tonsillar metastasis from lung cancer (adeno-squamous carcinoma). Tonsillectomy following radiotherapy was carried out, and bleeding symp-tom had disappeared and local recurrence was not recognized until he died of pancreatic cancer 3 months after tonsillar treatment was finished.

Whether major surgery including reconstructive procedure is necessary or not may need further discussion. But if the lesion is thought to be control-lable by tonsillectomy, positive surgical interven-tion should be performed because it may have the advantage to the maintenance of QOL.
