鼻・副鼻腔結核の1症例

Tuberculosis of Paranasal Sinus

I．はじめに

　鼻・副鼻腔結核症は抗結核剤が開発される以前の肺結核症が多かった時期でさえ発生頻度は低く，現在ではさらに稀な疾患となっている。今同原発性鼻・副鼻腔結核症の1例を経験したので，若干の文献的考察を加え報告する。

　A 23-year-old female patient developed left nasal stuffiness with purulent nasal discharge.The patient was referred to this department with diagnosis of unilateral sinusitis. The examination revealed that the base of the middle and lower concha was covered with white exsudate, and x-ray studies showed that the maxillary sinus was diffusely clouded.

　Caldwell-Luc's operation and ethmoidectomy and excision of the lower concha were performed. Gaseous substance was found in the posterior ethmoid sinus, and the membranous part of natural opening of the posterior ethmoid and maxillary sinuses appeared yellow with rather fibrous swelling. Pathological findings strongly suggested tuberculosis, and tuberculous bacilli were found after 8 weeks on post-operative culture. After the operation, INH, EB and RFP were administerd for 7 months. No recurrence has been noted.
