選択的塞栓術ならびに外頸動脈遮断により摘出した上顎洞血瘤腫の1例

Maxillary Hematoma:A Case Report

　はじめに

　上顎洞血瘤腫は臨床的な診断名で，上顎洞内で出血と器質化を繰り返し腫瘤状になった状態である。その臨床症状や画像所見などから，悪性腫瘍との鑑別が問題となることがある。また，上顎洞血瘤腫は，摘出術の際に出血をきたすことがあるため，その診断，治療には注意を要する。

　今回われわれは，上顎洞血瘤腫に対し術前に血管造影ならびに選択的塞栓術を施行，また術中外頸動脈を一時的に遮断して腫瘤を摘出した症例を経験したので，若干の文献的考察を加えて報告する。

A 16-year-old boy complained of unilateral nasal obstruction and nasal bleeding. CT scan showeddestruction of the floor of the orbit and MRI showed various intensity.

After angiograpy and selective embolization of the feeding arteries, and the external carotid arery was clamped the mass in the antrum was resected.

The preoperative angiography was effective in estimating the vascular proliferation in the hema-toma.
