I．はじめに

　いわゆる上顎洞血瘤腫は臨床的呼称であり，その本態については血管腫など腫瘍性変化とするものと炎症または出血による二次的変化とするものがありいまだ定説がない。症例によっては上顎骨の破壊を伴い，眼球突出，頬部腫脹，鼻出血など一見悪性腫瘍類似の所見を示し悪性腫瘍との鑑別が問題となる。また摘出時大量出血をきたす場合があり，その出血予防対策も考慮する必要がある。

　今回われわれは上顎悪性腫瘍を疑わせた本疾患の2例を経験した。これら2例につぎ病理組織学的検索と摘出時の出血予防について若干の検討を行ったので報告する。

　Hemangioma of the maxillary sinuses in two teenage girls are presented. Both showed marked bony destruction which made differential diagnosis from malignant neoplasms difficult. Histopathology revealed a pseudotumor consisting of extensive tissue necrosis, hemorrhage with abnormal vascularization and fibrosis with atypical fibroblastic reaction.

　Both patients underwent successful operations without any serious bleeding by utilizing the new hemostatic technique of superselective arteriographic embolization by creating an artificial embolus in the maxillary artery.

　For effective treatment of this disease, specificity in pathological diagnosis, prompt differentiation from other maxillary malignancies and effective control of bleeding must be sought for.
