眼症状を呈した蝶形骨洞病変の3症例

Three Cases of Sphenoid Sinus Cyst Presenting Eye Symptoms

　はじめに

　蝶形骨洞病変の初発症状は，蝶形骨洞と眼窩や頭蓋内の隣接した解剖学的位置関係から視力や視野，眼球運動などの眼症状が多く，耳鼻咽喉科的疾患でありながら鼻症状はあまり多くない^1）。そのため，眼科や脳神経外科など隣接臓器を取り扱う診療科からの紹介が多い疾患の1つである。眼症状の中でも特に，視力低下で発症した蝶形骨洞病変は，早急な診断治療を要する重要な副鼻腔疾患である。

　今回，われわれは眼球運動障害や視力障害をきたした蝶形骨洞病変の3症例に対して内視鏡的鼻内手術を施行し，良好な結果を得たので，若干の文献的考察を加えて報告する。

Three cases of the sphenoid sinus disease whichpresenting with disturbance of ocular movement or dysopia were reported.

In all cases, CT scan revealed a shadow in the sphenoid sinus, and the endoscopic sinus surgery (ESS) was performed. After operation the symp-toms were improved, but recovery of the vision depended on the period from the onset, the preoper-ative vision, the damage of the ocular fundus.
