ふらつきを主訴に来院したFisher症候群の3例

Miller Fisher Syndrome:Report of 3 Cases

　はじめに

　Fisher症候群は，1956年にMiller-Fisherが報告し，急性の外眼筋麻痺，小脳性運動障害，腱反射消失を3主徴とする一過性の予後良好な疾患である^1）。本症候群は，上気道炎や胃腸炎の1〜3週後に複視，歩行時のふらつきで発症し，多くは1〜2週をピークにその後徐々に回復する。純粋に3主徴のみ呈することは少なく，先行感染の病原体を認めること，軽度の筋力低下や感覚障害を伴う例が多いこと，Guillain-Barré症候群（以下，GBSと略）の診断基準を満たす例が1／3を占めること，電気生理学的に末梢障害を認めることからGBSの亜型とも考えられている^2）。一般に上記の3主徴を呈することは少なく，脳幹梗塞と誤診されることも少なくない。

　今回筆者らは，臨床所見および諸検査よりFi-sher症候群と診断された3症例を経験したので，若干の文献的考察を加えて報告する。

Miller Fisher syndrome is characterized by oph-thalmoplegia, ataxia and areflexia. We report 3cases of typical Miller Fisher syndrome with above symptoms.

All of them had serum antibodies against gang-lioside type GQ 1 b, which are specific for an im-muno-mediated neuropathy of the cranial nerves, especially in the oculomotor nerve.

Two of them needed no intensive treatment, how-ever, the other one received plasma exchange ther-apy. Clinical symptoms of ophthalmoplegia, ataxia, and areflexia were recovered within 6 months.
