Fisher's syndrome is an unusual form of acute idiopathic polyneuritis, and the diagnosis was made by the classical triad consisting of ophthalmoplegia, ataxia and loss of deep reflex.

A 64-year-old male patient presented with atypical Fisher's sydrome with vertigo, paralysis of cranial nerve VII, IX, X, XII and ataxic apnea, loss of memory in addition to triad. The neurotologic examination showed complications of cerebellar and brainstem lesions.
