めまいを主訴とした異型Fisher症候群の1症例

A Case of Atypical Fisher's Syndrome with Vertigo

　はじめに

　Fisher症候群は眼筋麻痺，運動失調，深部反射低下を3主徴とする，急性多発性神経炎の1亜型である^1〜3）。現在までの報告では，複視で発症し，上記3主徴が次第に完成し，最終的にFisher症候群と診断されている症例が多い^4,5）。今回われわれは，めまいを主訴とし，3主徴のほか意識障害，無呼吸など多彩な中枢神経症状を呈し，神経耳科学的検査から小脳および脳幹病変の合併が示唆され，異型Fisher症候群と診断した症例を経験したので報告する。

Fisher's syndrome is an unusual form of acute idiopathic polyneuritis, and the diagnosis was made by the classical triad consisting of ophthalmoplegia, ataxia and loss of deep reflex.

A 64-year-old male patient presented with atypical Fisher's sydrome with vertigo, paralysis of cranial nerve VII, IX, X, XII and ataxic apnea, loss of memory in addition to triad. The neurotologic examination showed complications of cerebellar and brainstem lesions.
