治療中に急性増悪したワルダイエル輪のリンパ腫型ATLの1例

A Case of Lymphoma Type Adult T-Cell Leukemia in Waldeyer's Ring

　はじめに

　耳鼻咽喉科領域ではワルダイエル輪の悪性リンパ腫は比較的多く経験する。B細胞性リンパ腫が多いが，T細胞性リンパ腫も少なくない^1）。ワルダイエル輪に発生するT細胞性悪性リンパ腫の中には成人T細胞白血病ウイルス（HTLV-I）により発症する成人T細胞白血病（ATL）のリンパ腫型（リンパ腫型ATL）があり，これらの症例は予後が不良である^2）。通常リンパ腫型ATLは白血化することなくリンパ腫として経過するとされている^3）。今回われわれは，治療中に急性増悪し白血化したワルダイエル輪のリンパ腫型ATLの1例を経験したので，若干の考察を加えて報告する。

A case of lymphoma type ATL in Waldeyer's ring showed acute leukemic change in the course of chemoradiotherapy in a 60-year-old male whocomplained of sore throat without any general symptoms for a month. The clinical examinations including pathological studies revealed the lesion to be lymphoma type ATL in the right palatine tonsil. A therapeutic course of multidrug chemotherapy followed by radiotherapy was performed. However, in the coures of radiotherapy, the lesion in Wal-deyer's ring showed acute leukemic change with general symptoms. The intensive chemotherapy failed to control the acute leukemic progress of the Waldeyer's lymphoma type ATL.
