　はじめに

　放射線による側頭骨壊死（radionecrosis）は晩発性の放射線障害として知られているが，いったん発症するとその治療に難渋することが多く，近年においても致命的な頭蓋内合併症を引き起こした症例が報告されている^1,2）。頭頸部悪性腫瘍の治療において放射線治療はしばしば用いられ，腫瘍の抑制に有効な役割を果たしているが，それに続発して起こり得る障害についても十分留置していくことが必要である。今回われわれは，中耳癌に対する側頭骨亜全摘の術前，術後の放射線治療後，20年目に発症した側頭骨壊死の1症例を経験したので，若干の文献的考察を加えて報告する。

Osteoradionecrosis of the temporal bone may occur as a result of radiation therapy for head andneck carcinomas or brain tumors. A 64-year-old female received radiation therapy for squamous cell carcinoma of the middle ear 20 years ago, and then she developed osteoradionecrosis of the temporal bone. The patient underwent extensive debridement with removal of sequestrations in the temporal bone and adjacent areas, and abscess drainage. Her post-operative course was satisfactory and there was no progression of the disease.
