放射線外照射後に生じた下顎骨壊死の1症例

A Case of Osteoradionecrosis of the Mandible

　はじめに

　口腔内腫瘍に対する放射線治療には外照射，組織内照射および両者併用療法があるが，照射治療後の骨壊死の発生率は5〜15％と報告されている^1）。しかし外照射単独治療に限定すると，その発生率は約1％と少ない^1,2）。

　今回われわれは，放射線外照射により生じた下顎骨壊死に対し，下顎骨区域切除，腹直筋による再建を必要とした症例を経験したので報告する。

A 70-year-old male with oral cavity carcinoma (T2NO) was given extra-irradiation therapy by atotal dose of 70 Gy. Eighteen months later, the mandibluar bone showed necrotic change and exposed itself in the oral cavity, and pathological fracture was also observed. Partial resection of the mandible was performed and the following defect was successfully reconstructed using a rectus ab-dominis flap.
