呼吸困難をきたした再発性正中頸嚢胞の1例

Recurrent Thyroglossal Duct Cyst Presenting Dyspnea:A Case Report

　はじめに

　正中頸嚢胞は甲状舌管の遺残組織より発生する先天性頸嚢胞の1つである。多くは舌骨を中心とした正中部に発生し，無痛性の頸部腫脹を呈するのが通例である。今回われわれは，呼吸困難を呈して来院した患者に喉頭蓋の膿瘍を伴った急性炎症を認め，これが再発性の正中頸嚢胞の感染が波及して生じたと考えられた症例を経験した。これは通常みられる正中頸嚢胞の症状とは極めて異なるので，症例を呈示するとともに，正中頸嚢胞の合併症について考察を加える。

A 49-year-old man presented with dyspnea due to the spread of infection originating from recurrent thyrogrossal duct cyst to the base of the tongue and epiglottis. The case is discussed by comparing with other case reports of unusual thyrogrossal duct cyst, and the importance of Sistrunk operation is emphas-ized.
