上咽頭癌再発例の検討

Studies on Recurrent Cancer of the Nasopharynx

　I．はじめに

　上咽頭癌（nasopharyngeal carcinoma，npc）は台湾を含む中国南部，香港などの住民に多発するという民族的特徴をもつとともに，ウイルス学的研究からは，Old（1966）の報告以来^1），Burkittlymphoma（BL），およびinfectious mononucleosis（IM）と同様に，患者はEpstein-Barr virus（EBV）に対する高い抗体価を有し，また上PW頭癌細胞がEBVを保有し，活性のあるEBVがrescueされたことにより^2），この癌もEBVによって発症するのではないかとの疑いがもたれ，発癌のメカニズムの追求が行われている。

　またこの癌は，呼吸器系における最も予後の悪い悪性腫瘍といわれ，諸家の報告でも5年生存率で25％前後との報告が多い。この癌の自覚症状は発現しにくく（silent area），また直接視診しにくい部位（clinical blind spot）であるため，早期発見が困難で，診断が確定したときには，すでに遠隔臓器に転移しており，治療の時期が失われている例も少なくない。なお臓器転移は放射腺治療中にも，また終了後にも起こり得るわけで，この疾患の予後の悪い最大の原因となっている。それゆえ上咽頭癌は局所的な治療だけでなく，免疫機能などを考慮に入れた全身的管理をも重視しなければならない。

　One hundred and seventeen cases of nasopharyngeal carcinoma (NPC), during past ten years (1970-79) at Yokohama City University Hospital, were investigated from the standpoint of relapse and prognosis. And the following results were obtained.

　1. The early diagnosis is desirable.

　2. NPC is regarded as the systemic disease. And the careful examination of the whole body is necessary for detection of distant metastasis. To prevent recurrence of tumor, the more rational therapeutic plan is indispensable.

　3. It is suspected that the immunological status of the NPC patients influences on their prognosis, and it is concluded that the more appropriate immunotherapy, such as the specific immunotherapy, should be the core of the therapy in future.
