放射線治療が著効した軟部好酸球肉芽腫症例

A Case of Eosinophilic Granuloma of the Soft Tissue Treated with Radiation Therapy

　はじめに

　軟部好酸球肉芽腫は最初の報告者の名にちなんで木村病と呼ばれ^1），耳下腺部や顎下腺部の皮下軟部組織に腫瘤を形成する疾患である。日本人をはじめアジア人に多く，青壮年男子に好発する。慢性に経過する無痛性の皮下腫瘤で，好酸球増多や血清IgEの上昇を伴い，アレルギー疾患の合併が多い^2）。これらの臨床所見や病理学的所見からも診断は容易であるが，治療は薬物療法，外科的治療，放射線療法を単独や併用で行うが再発も多いとされている^2）。今回われわれは，放射線療法と短期間のステロイド剤の併用が著効した軟部好酸球肉芽腫の1例を経験したので，若干の文献的考察を加えて報告する。

A 11-year-old man presented our hospital with a swelling of the left parotid region. Histopath-ological findings of the neck mass revealed eosino-philic granuloma of soft tissue. The peripheral blood counts showed an increase of eosinophiles (35%), and serum IgE level was extremly high (9,400 U/m/). The administration of antiallergic drug had no effect and the mass was enlarging. Therefore irradiation with 22.4 Gy was started in combination with steroid hormons. This was very effective and the patient made good progress after treatment. There was no recurrence to date of this report.
