口蓋垂軟口蓋咽頭形成術後に咽頭側壁膿瘍をきたした1例

Recurrent Lateral Pharyngeal Wall Abscess after Uvulopalatopharyngoplasty: A Case Report

　はじめに

　口蓋垂軟口蓋咽頭形成術（以下，UPPPと略）は，閉塞型の睡眠時無呼吸症候群の手術療法として広く行われている^1）。UPPPの術後合併症として現在までに報告されているものは，術直後には創部からの出血，創部の感染，気道閉塞が，遅発性の合併症としては鼻咽腔閉鎖不全，鼻咽腔狭窄，創部からの出血がある^2〜6）。しかし，術後数か月してから術創部，特に咽頭側壁に膿瘍を形成したという報告はみられない。今回他院にて，睡眠時無呼吸症候群の治療としてUPPPを施行された後，咽頭側壁に膿瘍形成を反復した1例を経験したので，特にその感染経路について検討を行い報告する。

Recurrent lateral pharyngeal wall abscess after uvulopalatopharyngoplasty was found in a 32-year-old male with a severe sore throat, dysphasia andfever. Needle aspiration of pus and chemotherapy were performed and this condition was improved.

Review of the literature disclosed that peritonsil-lar abscess after uvulopalatopharyngoplasty was uncommon. In this present case, because lateral pharyngeal wall abscess was repeated, nasopharyn-geal stenosis may be occurred, and strict follow up will be necessary.
