Abstract. Purpose:To report a case suspected of optic perineuritis and angiolymphoid hyperplasia with eosinophilia. Case:A 68-year-old woman with bronchial asthma presented with impaired vision in the right eye. She had been diagnosed with swelling of right lateral rectus following swelling of eyelid 13 years before. Visual acuity decreased to 0.01 right 5 years before. Pulsed corticosteroid induced normalization of visual acuity. Findings:Corrected visual acuity was 0.02 right and 1.2 left. The right eye showed relative afferent pupillary defect. Eosinophilia was present in the peripheral blood. Magnetic resonance imaging(MRI)showed nodular thickening around the right optic nerve,suggesting optic perineuritis with Kimura disease. Pulsed corticosteroid was followed by improvement of vision to 0.4 which decreased to 0.1 3 months later. Conclusion:This case is suspected of Kimura disease with optic perineuritis due to past history,presence of allergic status,and inflammation of external ocular muscles.
