悪性腫瘍の随伴性小脳変性症疑い例の神経耳科学的所見

Neuro-otological Findings in a Patient with Cerebellar Ataxia Accompanying Breast Cancer

　はじめに

　悪性腫瘍の浸潤や転移によらずに，いわゆるremote effect （遠隔効果）によって生じる神経筋疾患は，傍悪性腫瘍症候群（paraneoplastic synd-rome）と総称される。この傍悪性腫瘍症候群の1つで，悪性腫瘍のremote effectによって亜急性に小脳失調をきたす亜急性小脳変性症では，血清中の抗プルキンエ細胞抗体が，特異的に高力価で存在することが証明され^1,2），近年，腫瘍随伴性小脳変性症（paraneoplastic cerebellar degenera-tion）と呼ばれるようになってきた^3,4）。

　今回われわれは歩行時のふらつき，構音障害で発症し，臨床上，乳癌の遠隔効果による腫瘍随伴性小脳変性症が疑われた1症例を経験したので，その神経耳科学的所見について若干の文献的考察を加えて報告する。

A 73-year-old woman presented with gait distur-bance and dysarthria without family history. On examination she showed vertical positioning nys-tagmus, saccadic pursuit, ocular dysmetria, distur-bances of optokinetic nystagmus and visual suppres-sion in caloric testing, and marked dysequilibrium. A markedly elevated titer of anti-granuler cell antibody was detected and, thereafter, breast cancer on the left side was found. This case suggested that otoneurologists should consider the possibility of paraneoplastic cerebellar degeneration in mind when they see the aged patients with cerebellar ataxia without family history.
