鼻中隔原発奇形癌肉腫の1例

A Case of Teratocarcinosarcoma in Nasal Septum

　はじめに

　奇形癌肉腫（teratocarcinosarcoma）は，1984年にHeffnerら^1）によって確立された疾患概念で，特徴的な奇形腫様の組織像を示す悪性腫瘍である。成人の鼻副鼻腔に発生した本腫瘍の報告例は極めて少なく，われわれの調べ得た限りでは，これまでに海外で26例^1〜5），本邦では1例^6）の報告を認めるのみである。今回，われわれは鼻中隔に原発した奇形癌肉腫の1例を経験したので，その病理組織学的特徴と類似疾患との鑑別を中心に，若干の文献的考察を加えて報告する。

A 51-year-old man complained of left epistaxis, and a tumor was found in the left nasal cavity. The radiological examination showed a tumor shadow in the left ethmoid sinus and nasal cavity. Histo-pathological findings revealed teratocarcinosar-coma.

The patient was treated by skull base surgery and chemotherapy (CDDP+5FU), but local recurrence and pulmonary metastasis were appeared 3 months after. Therefore, salvage treatments, such as chemo-therapy (CDDP+5FU×1, MIT+VP-16+CDDP+ PSL×1), immunotherapy and irradiation (60Gy), were performed. The patient has been alive 16 months after the surgery.
