上顎洞に発生した類上皮血管腫の1例

Epithelioid Hemangioma in the Left Maxillary Sinus:A Case Report

　はじめに

　類上皮血管腫（epithelioid hemangioma，以下EHと略す）は，1969年にWellsら^1）によって初めて記載され，1983年にEnzingerら^2）が血管内皮細胞の形態学的特徴に着目して命名したまれな良性の腫瘤様病変である。本疾患は頭頸部に好発するが，上顎洞に発生することは極めてまれである。

　今回われわれは，左上顎洞に発生し骨破壊を呈したため悪性腫瘍を疑った極めてまれなEHの1例を経験したので若干の考察を加えて報告する。

A 68-year-old male suffered from a painless swel-ling of the left cheek for approximately one month. CT and MRI showed a mass in the left maxillary sinus destroyed the posterolateral bony wall of the sinus. Histological examination of an open biopsy specimen revealed that the mass con-sisted of vessels with plump endothelia containing vacuoles and infiltrate of chronic inflammatory cells. Four days prior to surgical removal of the tumor, transarterial embolization of the feeding vessels was performed. Clinical course was unevent-ful 6 months after the operation.
