2．当科における下咽頭癌の治療とその成績

Treatment and Prognosis of Patients with Hypopharyngeal Carcinoma

　下咽頭癌は上咽頭癌と並び頭頸部癌のなかでも最も予後の悪い疾病とされている。しかし化学療法，再建外科が発達してきた現在，照射療法・手術療法を含めた集学的治療によりその成績は向上してきている。1988年以来当科では下咽頭癌症例に対し，術前化学・照射療法後に切除術，即時再建術を組み合わせた集学療法を基本とした治療を行っている。しかし手術拒否，遠隔転移など様々な理由から保存的治療を行った症例も存在する。1988年以来当科で治療を行った症例の治療法，予後およびその背景などを検討し報告する。

Between 1988 and 1994, 73 patients with squamous cell carcinoma of hypopharynx were treated in our department. Out of the 73 patients, 43 were operated after chemotherapy and/or radiation (group 1) and 30 were treated without surgery by reason of radical irradiation inoperable cases and refusal of surgery (group 2) . In group 1, 4 patints were stage 2, 10 patients stage 3 and 29 patients stage 4. In group 2, 2 patients were stage 1, 2 patients stage 2 and 26 patients stage 4. Five year cumulative survival rate for group 1 was 35.7%, while the survival rate for group 2 was 7.1%.
