当科における舌・口腔底癌の臨床統計—治療成績の検討を中心に

Statistical Analysis on Carcinoma of the Tongue and Floor of Mouth in Our Department:The Role of Elective Neck Dissection

　はじめに

　口腔癌のなかで舌癌，口腔底癌は発生頻度が高く，進行例においては舌と口腔底の両領域にまたがる例も散見される。舌および口腔底に限局した比較的小さな癌は，局所への組織内照射により，その多くは制御可能である^1）。しかし，それに携わる人の被曝線量の問題や設備の点から，組織内照射治療に際しては，施設によって制約を受けざるえない。一方，進行例は頸部リンパ節転移の出現率が高く，またその制御が難しいため^2），再建を伴った手術法が選択されることが多い。当科における舌癌および口腔底癌の治療は，adjuvant chemotherapyと手術療法を主体としてきた。今回，当科で治療した舌癌および口腔底癌症例をまとめ，主にその手術方法について考察する。

We have treated fourty-one patients with car-cinoma of the tongue and the floor of mouth.

All but one cases received surgical therapy with-out needle irradiation therapy with radium or others. The operative procedures were divided into three types: 1) partial glossectomy or resection, 2) combined resection using elective neck dissection and 3) combined resection with immediate thera-peutic neck dissection. The accumulated five-year survival rate was 68% in all cases. The role of the elective neck dissection in T2N0 cases was espe-cially discussed.
