はじめに
頭頸部領域に悪性リンパ腫が原発することは多いが,側頭骨に原発することは稀である。われわれが渉猟し得た国内外の1976年以降の報告数は,10例1〜8)であった。今回われわれは,側頭骨原発と考えられる悪性リンパ腫の1症例を経験したので,若干の文献的考察を加えて報告する。
Malignant lymphoma of the temporal bone is very rare. A 66-year-old woman complained of right ear fullness, otalgia, facial palsy, dizziness, dysphagia and hoarseness. The clinical examination revealed a gray and fragile tumor obstructing the right external auditory meatus, and extensive cra-nial nerve palsies on the right side like Garcin syndrome. CT scan and MRI showed a large soft tissue mass extremely destructing the right tempo-ral bone. Pathologocal diagnosis was malignant lymphoma (B cell, diffuse large cell, immunoblastic type). A clinical staging work up presented no other source of lymphoma, that corresponded to stage I of the Ann Arbor's classification. So, radiation ther-apy was done for 50Gy. However, the lymphoma disseminated, though primary tumor partially reduced and resulted in 4 months survival following the treatment.
