異常眼球運動を示した転換型ヒステリーの1症例

Convergence Hysteria presenting Abnormal Eye Movements

　はじめに

　精神分裂病，躁うつ病，ヒステリーなどの精神疾患で観察されるさまざまな異常眼球運動について，数多くの報告^1〜3）があるが，実際の眼球運動を記録したものは少ない。今回，初診時の異常眼球運動所見より中枢性めまいが疑われ，最終的に転換型ヒステリーと診断され，精神医学的アプローチによって，めまいおよび異常眼球運動の消失をみた症例を経験したので，その異常眼球運動の記録を示すとともに，文献的考察を加えて報告する。

A 27-year-old woman complained of unsteadiness with convergence hysteria and presented with differ-ent abnormal eye movements. Eye tracking test revealed saccadic pattern. Alternative eye wathing was not fair. And “involuntary” voluntary nystag-mus like eye movement was recorded at head positioning. The character of this eye movement was of saccades. At last apparent vertical eye movement observed only with Frenzel's glass, but not at gaze. After psychological therapy, they were normalized and she was released from unsteadiness.
