結膜原発悪性リンパ腫の1症例

A case of primary conjunctival malignant lymphoma

　45歳女性が，1年前からの左下眼瞼の腫瘍で受診した。左眼の球結膜に鮭紅色の腫瘤があり，生検でびまん性小細胞型悪性リンパ腫と診断した。免疫組織化学的にCD20とCD79aが陽性でB細胞性であり，PCR法による分子生物学的検索で単クローン性を示した。磁気共鳴画像検査（MRI）と⁶⁷Gaシンチグラフィによる全身検索で左眼結膜以外には異常がなく，結膜に原発した悪性リンパ腫と診断した。病理組織所見，臨床病期，患者の希望に基づき，総線量48Gyの放射線単独照射を行い，腫瘍は縮小した。以後2年半，局所再発と転移はなく，経過は良好である。

A 45-year-old female presented with swelling of her left lower eyelid sincelyear before. A salmon -pinktumor was present in the bulbar conjunctiva in the affected eye. Biopsy led to the diagnosis of diffuse malignant lymphoma of small cell type. Immunohistochemically, the tumor cells were positive for CD20 and CD79a. Molecular biological analysis using a polymerase chain reaction showed B-cell monoclonality. The tumor was diagnosed as primary conjunctival malignantlymphoma after no systemic involvement was detected by magnetic resonance imaging and gallium scintigraphy. Thetumor regressed following radiation totalling 48 Gy. She has been doing well with no local or systemic metastasis duringthe ensuing 30 months.
