重篤な縦隔洞炎を起こした深頸部感染症の1例

Mediastinitis Secondary to Deep-Neck Infections

　はじめに

　抗生剤や画像診断の発達した現在では，深頸部感染症から縦隔洞に炎症が波及することは，早期の抗生剤投与と外科的処置により少なくなっている。しかしながら，われわれは，糖尿病と閉塞性動脈硬化症の基礎疾患をもつ患者で，急性扁桃炎から深頸部感染症を生じ，抗生剤投与と頸部に外科的ドレナージを行ったにもかかわらず，さらに急速に縦隔洞炎を生じ，極めて重篤な状態に陥った患者を経験した。経過中に，細菌性ショック，膿胸，肺炎の併発，閉塞性動脈硬化症の悪化による右足壊死がみられたが，集中治療室管理として無事救命し得たので，この経過を文献的考察を加え報告する。

A 69-year-old patient developed deep neck infec-tion following to acute tonsillitis. In spite of trans-cervical drainage and antibiotic treatment, severe mediastinitis was occurred secondary to deep neck infection. The neck, mediastinum, and thorax were drainaged and irrigated with povidone iodine twice a day, in combination with antibiotic treatment. The underlying factors why the mediastinitis was advanced rapidly were thought to be old age, diabet-ic mellitus, and obstructive arteriosclerosis. Three months after admission, however, he was complete-ly recovered and discharged.
