縦隔洞膿瘍を併発した深頸部感染の1症例

Deep Neck Infection Complicated with Mediastinal Abscess:Report of a Case

　はじめに

　深頸部感染が縦隔まで波及することは比較的まれなことと思われるが，咽喉頭の感染から深頸部感染へ移行し，さらに縦隔へ感染が及ぶ場合がある。抗生物質が発達した現在においても，縦隔に炎症が及ぶと高い死亡率となっており，われわれ耳鼻咽喉科医は深頸部感染を管理するにあたっては的確な判断と対処が要求される。われわれは，上咽頭の炎症が咽頭後壁に波及し，咽後隙の深頸部感染から縦隔洞膿瘍へ移行した症例を経験した。縦隔洞膿瘍を早期に診断し，縦隔ドレナージを施行したため救命し得たので，ここに報告する。

Mediastinal abscess secondary to descending deep neck infection is rare. A 16- year-old man developeda mediastinal abscess secondary to descending retropharyngeal infection from adenoiditis.

A lateral neck x-ray revealed enlargement of the retropharyngeal space and few days later a CT scan revealed a mediastinal abscess. The mediastinal abscess was drained transcervically and the patient improved rapidly. We stress the possibility of the mediastinitis secondary to deep neck infection despite of any treatments.
