シスプラチンによる聴力障害について—とくに年齢との関係について

Hearing Loss after Cisplatin Treatment:Its Relationship to Age

　緒言

　1965年，Rosenbergによって発見されたシスプラチン（cis-platinum （II） diammine dichlor-ide：以下CDDPと略す）は，C₁₂H₆N₂Ptの化学式を有する白金化合物で，その作用機序がアルキル化剤と類似しており優れた抗腫瘍作用を示すため，泌尿器科領域，婦人科領域や耳鼻咽喉科領域などの悪性腫瘍に広く利用されている。CDDP投与による副作用としては，腎毒性や骨髄機能抑制などのほかに耳鼻咽喉科的には難聴，耳鳴などの聴覚障害が発症するとされている。聴覚障害については，Helsonら^1），Reddelら^2），Nakaiら^3）やその他多くの者により発現頻度，障害をきたす投与量，障害の性質や側頭骨標本による障害部位などについての報告がみられる。CDDPは小児から老人まで幅広く使用される薬剤であるため，年齢による聴力変化についても検討する必要性があると考えられる。しかしそれについての報告は少ない。今回CDDPを使用した腫瘍症例を対象として年齢と聴力障害について検討したので報告する。

An investigation was made into the relationship between hearing loss and age in 67 patients with malignant tumors aged 4-83 years who were treated with Cisplatin.

1) The younger the patients, the higher the inci dence of hearing loss. But it depended on the total cumulative dose of Cisplatin than the aging.

2) When the patients with aggravated hearing loss were divided into the under 19, 20-49 and over 50 age groups, it was revealed that hearing loss in high frequency range, especially 8 kHz was progres-sively severer with decreasing age.

Based on the results of this study and a literature review, it may be concluded that the incidence and severity of Cisplatin induced hearing loss increase with decreasing age.
