口腔底に発生した巨大類表皮嚢胞の1症例

A Large Epidermoid Cyst of the Floor of the Mouth

　はじめに

　頭頸部の類表皮嚢胞は，一般的に胎生期における第1・2鰓弓の癒合線上に遺残した外胚葉組織の迷入，または外傷・炎症・手術などによる上皮組織の迷入により生じる疾患である。本嚢胞は頭頸部の各所に発生しうるが，口腔領域においては，特に口腔底に発生したものが多く報告されている^1,2）。今回，われわれは，口腔底に発生した巨大な類表皮嚢胞の1例を経験した。MRIにより嚢胞と口腔底筋群の位置関係を診断することが本症例の術式の選択の際に有力であったので，文献的考察を加えて報告する。

MRI showed a cyst in sublingual region above the mylohyoid muscle. The cyst was excised by oral approach. We concluded that MRI was of great benefit in the diagnosis of lesions of the head and neck. Fifty cases of epidermoid cyst reported from 1965 to 1994, were reviewed and analysed according to size, location and surgical methods.
