甲状腺腫瘍術後に腕頭動脈破裂をきたし救命しえた1例

Rapture of the Innominate Artery after Total Thyroidectomy

　はじめに

　腕頭動脈破裂は頭頸部外科の重篤な術後合併症であるが，救命しうることは少ない^1）。これは，気管壁の感染が，動脈壁に波及し，気管腕頭動脈瘻を形成するために破裂が起こることが多く^1〜9），腕頭動脈が広範に脆弱化し，結紮切断，直接縫合，パッチ，人工血管置換などを行っても再出血するためである^1）。

　今回われわれは，甲状腺癌の縦隔および気管浸潤症例で，腫瘍切除と永久気管孔造設後6日目に腕頭動脈破裂をきたしたが，迅速な対処のもと破裂部位を直接縫合したのち縫合部位をテフロンパッチにて補強し，さらに大胸筋皮弁により術創を保護したことで救命しえた1例を経験したので若干の文献的考察を加えて報告する。

Rapture of the innominate artery is one of the fatal complications of tracheostomy. A patient in whom the innominate artery was raptured after total thyroidectomy and tracheostomy was treated successfully by surgical closure of a defect of the innominate artery using Teflon patch. The patient survived without rebleeding or tumor recurrence 2 years after the operation.
