　はじめに

　血管性の頸部腫瘤には血管腫，動静脈奇形，動脈瘤，頸動脈小体腫瘍に加えて，稀ではあるが動脈蛇行症の報告が散見される^1〜5）。これらの血管性病変に対して穿刺細胞診を安易に行うことは危険であり，触診や各種画像診断により確定診断をする必要がある。頸部動脈蛇行症で最も多いのは総頸動脈または内頸動脈に発生するものであるが，これに対して，鎖骨下動脈，腕頭動脈のものは極めて報告例が少ない。今回われわれは腕頭動脈蛇行症の1例を経験したので，本疾患における各種画像診断の長所・短所についての考察を含めて報告する。

A rare case of tortuous brachiocephalic trunk in a 75-year-old woman who presented with a 1-year-history of foreign body sensation of the throat without dysphagia or dyspnea was reported. An egg-sized pulsating mass was palpable in her supraster-nal region. CT and ultrasonic tomography showed a pretracheal mass lesion. The definite diagnosis was made by digital subtraction angiography which revealed a tortuous brachiocephalic trunk stretched out to the anterior neck. On the contrary, MR angiography failed to depict the shape of the trunk. The patient has been under the medical treatment for hypertension and hyperlipemia.
