気管切開を繰り返しカニューレ抜去困難をきたしたクルーゾン病の1例

Subglottic Stenosis after Tracheostomy in Crouzon's Disease

　緒言

　クルーゾン病は早期頭蓋骨癒合，上顎骨形成不全，眼球突出を主訴とする発育不全症候群で，約半数に常染色体優性遺伝を認める^1）。

　本疾患では，上顎骨の低形成のため気道狭窄が問題となることがある^2）。そのため気管切開をうける可能性があるが，本疾患と気道狭窄，気管切開についての詳細な報告は少ない。今回，われわれは気管切開を繰り返しカニューレ抜去困難をきたしたクルーゾン病の1例を経験した。狭窄部の病理所見から本疾患における気道狭窄発生の原因を考察し，本疾患における気管切開時の注意点に言及する。またカニューレ抜去困難の治療にRotary door flap法が有用であったのでその方法も併せて紹介する。

A 64-year-old woman with Crouzon's disease was treated well by “trough method”. Rotary door flap method was applied for closing a trough anteriorly.

Tracheostomy was performed 3 times in other hospitals.

Following the latest tracheostomy, she suffered from subglottic stenosis. In patients with Crouzon's disease, air way is often obstructed because of concomitant nasopharyngeal stenosis and hypo-plasia of the maxilla with craniosynostosis and frog like facies. The literature was reviewed and it was discussed that in craniofacialsynostosis like Cro-uzon's disease tracheostomy should be operated more carefully.
