頸部リンパ節転移をきたした肝細胞癌の1例

A Case of Hepatocellular Carcinoma manifested as Anterior Neck Mass

　はじめに

　頸部腫瘤が悪性の場合，頭頸部領域原発の悪性腫瘍の頸部リンパ節転移である可能性が高い。しかし，原発部位が肝であったという報告は非常に少なく，また肝細胞癌が頸部リンパ節に転移を示す例も非常に少ない。今回われわれは，前頸部腫瘤が主訴となった肝細胞癌のまれな1例を経験したので，若干の文献的考察を加えて報告する。

A 77-year-old male with hepatocellular car-cinoma (HCC) presented with an anterior neck mass. A tentative diagnosis was advanced laryngeal cancer or thyroid cancer. However, a fine needle aspiration biopsy followed by incisional biopsy revealed that his neck mass was HCC. Hepatic lesion was small and no other metastatic lesions were found. Differential diagnosis of a cervical lymph node swelling and HCC metastasis was dis-cussed.
