上顎癌手術17年後にみられた内頸動脈瘤破裂による大量出血症例

Massive Epistaxis from Aneurysm of the Internal Carotid Artery

　はじめに

　耳鼻咽喉科の救急患者の疾患として鼻出血はかなり大きな割合を占めるが，内頸動脈瘤破裂による鼻出血は極めてまれである。しかし生命の危険を伴う鼻出血であり，常に念頭に置くべき疾患である。今回われわれは，上顎癌全摘出術をうけ，17年後に上顎摘出部よりの大量の出血にて発症した内頸動脈瘤症例を経験したので，若干の文献的考察を加えて報告する。

Traumatic pseudoaneurysm of intracavernous internal carotid artery (ICA) is a very rare but life-threatening cause of epistaxis. A 56-year-old man was admitted to our hospital because of massive epistaxis occuring 17 years after resection and radiotherapy for maxillary cancer. The angiograms of the carotid artery revealed an aneurysm located at the intracavernous ICA. He was successfully managed by the detachable balloon occlusion of the ICA.
