頭部打撲後の顔面神経麻痺を初発症状とした乳児の神経芽腫（副腎原発）の1症例

Neuroblastoma in Infant manifested after Head Trauma and Facial Paralysis

　はじめに

　乳幼児の顔面神経麻痺は，日常臨床において時々遭遇する病態であるが，成人とはやや趣を異にする点が少なくない。先天性麻痺を代表とする小児特有の病態があり，また中耳炎のほか，腫瘍，血液疾患など重篤な疾患による症候性麻痺が少なくないことに注意が必要である。

　最近われわれは，頭部打撲後の顔面神経麻痺を初発症状とし，当初は外傷性麻痺を思わせたが，その後の検索により副腎原発の神経芽腫であることが判明した症例を経験したので報告する。

A 6-month-old male infant with neuroblastoma developed facial paralysis after head trauma. The patient fell down and struck his head against a floor. Two days later a swelling of the left temporal region and left facial paralysis were observed. The external auditory meatus was swollen and the tympanic membrane could not be seen. Traumatic facial palsy was considered, but after clinical inves-tigations neuroblastoma of the right adrenal gland with temporal bone metastasis was diagnosed.
