側頭骨疣状癌（Verrucous carcinoma）の2例

Verrucous Carcinoma of the Temporal Bone:Report of Two cases

　はじめに

　疣状癌（verrucous carcinoma，以下VCと略）は高分化型の扁平上皮癌の亜型と考えられるが，以下のような特徴をもって他と区別される。すなわち，1）外見が疣状あるいは乳頭状で表面に角化物の附着が著しい。2）肥厚した重層扁平上皮の構造を保ち，深部基底層近くでも角化する分化異常はみられるが，細胞の異型性や核分裂像が少ない。3）先端の太くなった細胞巣をつくりながら深達性に増殖する。4）進行がゆるやかで，局所再発はあるが，転移は稀である。

　VCは口腔や喉頭に多いが，食道，鼻腔，外陰部，手足などでもみられる^1）。側頭骨での報告は少なく，英語の文献では8例^2〜4），わが国では1例^5）の報告があるばかりである。

　一方VCに組織学的に類似するkeratoacanth-omaが耳でみられたとする2例の報告が最近あり^6,7），これがVCと異なるのか検討を要すると思われた。さらにひろく高分化型の扁平上皮癌とVCではどのようなちがいがあるかなど，いくつかの疑問がおこった。

　最近われわれの施設で，側頭骨のVCと診断のつけられた症例を2例つづけて治療する機会があったので，これらの症例から上記の疑問を検討してみた。

Two cases of verrucous carcinoma, one in the postoperative ear of an 80-year-old female, and the other in the external auditory canal of a 56-year-old male were reported. Repeated examinations by pathologists and careful consideration of all clinical informatios may be necessary for proper diagnosis, as was the case in the female patient, who had five pathological examinations before the final diagno-sis was made. The clinical and histopathological features were important in differentiating this dis-ease from other squamous cell carcinoma and ker-atoacanthoma.
