上顎洞根本手術と同時に行う口腔上顎洞瘻の閉鎖術について—フィブリン糊を用いた簡便な方法の試み

Closure of Oro-antral Fistula Using Fibrin Glue and Bone Fragments

　はじめに

　抜歯の際の穿孔から移行した口腔上顎洞瘻孔のうち耳鼻咽喉科に紹介される例は，上顎洞炎を併発しており，上顎洞根本手術を必要とすることがほとんどである。われわれは，上顎洞根本術と口腔上顎洞瘻孔の閉鎖を同時に行う場合は下鼻道対孔粘膜弁を利用する方法を用いてきたが，1）瘻孔が大きい際には十分覆うことができない，2）粘膜弁がずれることがある，といった欠点があった。周囲の健常な部分からの有茎粘膜弁を利用する方法（頬粘膜弁法，口蓋粘膜弁法，舌弁法）は，確実な方法といえるが，上顎洞根本術の手術範囲の組織を利用して他の健常部分を傷つけずにすませる方法がより望ましい。

　われわれは平成3年（1991年）1月以来，8例に対して上顎洞を開窓する際に上顎洞前壁より削除した骨片を，腐骨や肉芽を除去した後の瘻孔に詰め込んでフィブリン糊で固定する方法で口腔上顎洞瘻孔の閉鎖を行い，良好な結果を得ることができたので，報告する。

We described technique using fibrin glue and bone fragments for closure of an oro-antral fistula.

Eight patients were surgically treated for odontogenic sinusitis, complicated with all oro-antral fistula following tooth extraction, using Caldwell-Luc procedure and fistula closure. After removal of the whole mucosal lining, the fistula was obturated with fibrin glue and bone fragments that were collected from fenestration of the antrum. In all eight cases, the oro-antral fistula was closed without recurrence.

It was concluded that this technique can be safely applied for closure of the oro-antral fistula.
