鼻内視鏡手術と同時に行う口腔上顎洞瘻閉鎖手術の術式について

Simultaneous Surgery of Endoscopic Sinus Surgery and Closure of Oroantral Fistula

I．はじめに

　歯性上顎洞炎に対しては，原因歯と上顎洞炎を改善させる治療が行われる。上顎洞根本手術における歯頸部切開のアプローチは，同一術野で原因歯の抜歯と口腔上顎洞瘻の閉鎖が可能な手術法として報告されている^1）。また，鼻内視鏡手術が普及しその有用性が確立した昨今では，まず鼻内視鏡手術のみを行い，原因歯の治療は必要に応じ二次的に行う方法などが報告されている^2,3）。歯性上顎洞炎に対する手術療法では，手術侵襲や患者負担の軽減を考慮すると，鼻内視鏡手術と原因歯に対する手術を同時に行うことが望ましい。

　そこで今回，原因歯の抜歯が必要な歯性上顎洞炎患者に対する鼻内視鏡と口腔上顎洞瘻閉鎖の同時手術について報告する。

　We described the simultaneous surgery of endscopic sinus surgery and closure of oroantral fistula in patients with odontgenic maxillary sinusitis. Closure of oroantral fistula was performed immediately after endscopic sinus surgery. Mucoperiosteal flap was used for closure of oroantral fistula in patients without the donor tooth. In patients with the donor tooth,tooth autotranslation was performed. This simultaneous surgery is an easy,minimally invasive procedure in patients with odontgenic maxillary sinusitis.
