頭痛診療における耳鼻科医の新しい役割

A Role of E.N.T. Physicians for Diagnosis and Treatment of Headache

　耳鼻咽喉科外来に頭痛を主訴として訪れる患者は多い。片頭痛，緊張型頭痛，群発頭痛のような機能的頭痛もかなり多いが，耳鼻科本来の疾患である副鼻腔炎，アレルギー性鼻炎および中耳疾患などに基づく器質的頭痛もかなり多く，これらについては従来より述べられているところである^1）。

　今回，私どもは従来より原因が解明されなかった頭痛のうち，画像診断の進歩によって早期診断が可能となった蝶形骨洞病変および小児副鼻腔炎に基づく頭痛およびその治療について述べ，さらに最近，P．Bonaccorsiにより提唱され始めた“primary headache”に対する“neurovascular de-compressive septo-ethmoid-sphenectomy”について言及し，諸賢のご参考に供したい。

Headache is one of most serious subjects for otorhinolaryngologist to differentiate its etiology.

Nowadays the improvement of the imaging technique (CT, MRI) enables us to establish a correct diagnosis and to give a suitable treat-ment for headache.

In this paper the authors have described head-ache that comes from silent diseases of the sphenoid sinus (cyst, purulent sinusitis) and sinusitis in children who complain of only headache.

Furthermore, we have described the neuro-vascular decompressive septo-ethmoid-sphenectomy, proposed by P. Bonaccorsi for so-called “primary headache”.
