小児副鼻腔炎に対する三者併用療法—小児副鼻腔炎治療における新しい展開

Triple Combined Therapy against Sinusitis in Children

　はじめに

　私どもが行っている，小児副鼻腔炎に対する中鼻道経由キリアン洗浄管を用いて行う上顎洞洗浄法（以下，上洗と略）は，特に限局感染型およびその合併症であるいわゆる「原因不明熱」「原因不明頭痛」「副鼻腔気管支症候群」「化膿性上顎洞炎を伴ったアレルギー性鼻炎，気管支喘息および滲出性中耳炎」に著効を示すことは従来より発表してきたとおりであり^1,2），症例は2,000例を超している。

　しかし，対象を限局感染型に限定し，起炎菌に対する第一選択剤とされるアンピシリン系抗生剤投与下に上洗を行っても，その20％は全く効果を示さなかった。この20％の難治例，すなわち10回，20回，30回と洗っても多量の膿汁が排出される例に対して，私どもは，その対策に苦慮していた。しかし，ニューマクロライド（CAM・RXM）を使用しながら上洗を行えばさらに治癒率がよくなること，最近，さらに塩酸アゼラスチン内服の併用，すなわち上洗，ニューマクロライド内服，塩酸アゼラスチン内服の「三者併用療法」を行えば，さらに治癒率が向上することを知ったので報告する。これは小児副鼻腔炎治療における新しい展開といえる。

For the last ten years, we have performed antral irrigation using a Killian's cannula through the middle nasal meatus in over 2000 cases of sinusitis in children. Excellent results were achieved in 20 % of the cases.

In these cases, we found that irrigation and simul-taneous administration of new macrolide, such as CAM and RXM, were more effective than irrigation only. Furthermore, adminstration of Azelastine with these therapies showed remarkable results. We named this therapy Triple Combined Therapy.

We consider that new macrolide and Azelastine have antibiotic, anti-allergic effects, and also im-muno stimulating activities.
