圧迫眼振の三成分解析

Three Components Analysis of Fistula Nystagmus

　はじめに

　われわれは，これまでにコンピュータ画像認識の技術を応用した新しい眼球運動解析方法で眼球運動の回旋成分を含む三成分の解析を行い，報告してきた^1〜4）。この方法で，温度眼振のように内耳から誘発される眼振の水平，垂直，回旋成分を定量解析することで，単一半規管から誘発される眼球運動との関連から^5〜7）その眼振発現メカニズムを推察することが可能である^8〜9）。

　圧迫眼振も内耳から誘発される眼振である。すなわち，圧迫眼振は内耳瘻孔への圧刺激によって誘発される眼振であり，その発現メカニズムはEwaldの法則に従って考えられてきた。最近では，McCabc^10）が，臨床例の検討で，圧迫眼振は内耳瘻孔の存在部位により眼振の種類，方向が異なるために術前の注意深い観察によって瘻孔部位を正確に診断することが可能であると述べている。しかし，圧迫眼振には非定型的瘻孔症状の見られることもあることから，その発現メカニズムはEwaldの内リンパ流動説に対して内リンパ腔内圧の変化によるという報告^11）もあり，一致した見解を得ていない。

In order to clarify the role of three semicircular canals on generation of fistula nystagmus, three components (horizontal, vertical, and torsional) of fistula nystagmus were investigated using com-puterized image recognition technique, since each semicircular canal induces its specific eye movements, including torsional components.

In patients with a lateral canal fistula, it can be said that the pressure stimulation to the fistula causes the endolymphatic How and stimulates all three semicircular canals. Whereas, in patients with fistulae in addition to the lateral canal, the characteristics of fistula nystagmus cannot be explained by the endolymphatic convection alone.
