内耳奇形に脈絡膜欠損症を合併し，混合性難聴を呈した小児の1例

A Case of the Inner Ear Anomaly with the Choroidal Coloboma Presenting the Mixed Hearing Impairment

　はじめに

　内耳奇形は近年の画像診断の技術の進歩により診断が容易になってきた^1）。内耳奇形のタイプ，またそれに伴う臨床症状は症例ごとに様々であることも知られてきた^2）。したがって，難聴，平衡機能の評価，さらにしばしば合併する他部位の先天性異常に対して，1例1例充分な検討をする必要があると考えられる。われわれは内耳奇形を伴う難聴に先天性脈絡膜欠損症を合併し，さらに成長してからの聴力検査にて混合性難聴であることが判明した小児の1例を経験したので報告する。

An 8-year-old boy with severe hearing loss and dysequilibrium was reported. Pure toneaudio-metry revealed a mixed type of hearing impair-ment, in spite of no finding of otitis media with effusion. He had no response to thermal stimu-lation in either ear, but he showed rotatory nystagmus. CT scan revealed aplasia of the semicircular canals in both sides. Furthermore, ophthalmological examinations showed choroidal coloboma. This case suggested us that we should take disturbance of sound conduction into con-sideration even in patients with anomalies of the inner car.
