 A 17-year-old male, with bilateral mixed hearing loss, left cochlear deformity, bilateral congenital cervical fistulas left preauricular fistula, bilateral microtia and receding jaw was presented.

 Exploratory tympanotomy revealed following anomalies: 1) The stapes was missing and the oval window was closed by membranous tissue. 2) The long process of the incus was shortened and at its tip a mass of connective tissue (possibly residual stapes) was attached. 3) The stapedial muscle was seen as a string of connective tissue between pcsterior bony tympanic rim and oval window. On tomograms of the temporal bone, the left cochlea presented cystic view.
