ワルダイエル輪を主病変としたT細胞型悪性リンパ腫の3例

Three Cases of T-cell Lymphoma in Waldyer's Ring

　はじめに

　頭頸部の悪性リンパ腫は節性と節外性とに大別され，節外性リンパ腫の部位としてはワルダイエル輪が最も多く，鼻副鼻腔がそれに続いている。病理学的に悪性リンパ腫はホジキン型と非ホジキン型に分類され，本邦に多い非ホジキン型はさらに免疫組織学的にはB細胞リンパ腫とT細胞リンパ腫に分類される。ワルダイエル輪に発生する悪性リンパ腫はほとんどがB細胞性であるが^1），T細胞性の報告もある^2）。

　われわれはワルダイエル輪を主病変としたT細胞リンパ腫の3例を経験した。いずれも受診時にはワルダイエル輪内転移や全身への転移を伴っていたが，扁桃病変が慢性炎症と思われたり，皮膚の結節性紅斑様皮疹が扁桃炎関連病変と思われるなど，視診だけでは診断を誤りかねない症例であったので報告する。

Three cases of stage III malignant T-cell lym-phoma, which mainly involved the Waldyer's ring were reported. On microscopic examination, one case showed IBL-like T-cell lymphoma and the remaining two showed malignant lymphomas, diffuse, mixed cell type, according to LSG classi.fication. The bilateral palatine tonsils were in-filtrated lymphoma cells, in addition to the lingual and pharyngeal tonsils. The skin lesion that resembled erythema nodosum was accom-panied in two cases. T-cell lymphoma involving mainly palatine tonsil has a tendency of an early spread in the Waldyer's ring and whole body, compared with B-cell lymphoma.
