はじめに
近年,人口の高齢化現象に伴い,非ホジキンリンパ腫(non-Hodgkin's lymphoma, NHL)は他の悪性腫瘍と同様,高齢者の増加傾向を示している1)。しかし高齢者NHLを対象とした研究は少なく2),治療と予後因子との関係においては未だ不明の点がある。
著者らは1983年9月から金沢大学医学部附属病院内で,内科,放射線科,耳鼻咽喉科,病理を中心とした悪性リンパ腫の検討会を発足させ,共通のプロトコールで診断ならびに治療法検討を行っているが3〜5),今回ここに登録されたNHLのうち,ワルダイエル輪原発33例中,高齢者(75歳以上)10例の治療成績を中心に検討を加えたので報告する。
Non-Hodgkin's malignant lymphoma (NHL) of Waldeyer's ring in aged patients (older than 75) were studied. There were 10 cases in aged group among 33 patients with NHL of Waldeyer's ring in 275 cases of NHL. Histologically diffuse type was 84.8% in Waldeyer's NHL according to LSG classification, and most of them were large cell type. The rate of complete remission in patients with clinical stage II was 57.1% in aged group and 93.8% in patients less than 75. The five years survival rate showed 35% in aged group compared to those of 100% in other group. The reason of poor prognosis in aged group was a failure of radiation therapy as a primary treatment for the patient with clinical stage II.
